Provider resources

Getting started with ALW provider enrollment.

A practical starting point for California care-home owners, with links to the state’s current forms and requirements.

First, make sure you are on the right path.

This page is for facility owners exploring ALW provider enrollment. RCFEs are Residential Care Facilities for the Elderly; ARFs are Adult Residential Facilities. People seeking care for themselves or a family member should use the DHCS participant resources.

Start with the official requirements

DHCS describes provider licensure, good-standing and operational requirements. It also publishes the enrollment forms and submission instructions. Read the current version before relying on a saved checklist or example.

A practical readiness checklist

  • Identify the application type. An initial application, ownership change and location change may require different steps.
  • Check your license and location. Review your facility’s current standing and whether it is in a participating county.
  • Gather the actual records. Organize entity, licensing, staffing, insurance, property and operational documents relevant to your application.
  • Resolve inconsistent details. Check that names, roles, addresses and dates agree across the evidence.
  • Plan for review and signatures. Know who must confirm facts, sign documents and complete any required notarization or fee arrangements.

This checklist helps you organize. It is not an eligibility determination or a complete substitute for the state’s instructions.

Three distinctions that prevent confusion

Provider enrollment and resident eligibility

A facility’s enrollment is different from a person’s eligibility for ALW services. Care Coordination Agencies have a role in participant assessment and coordination. Enrollment does not guarantee resident referrals, occupancy or revenue.

Application preparation and state review

GetALW’s 2–3 week preparation estimate begins after required documents are received and reviewed. The agency’s review happens separately. GetALW does not control that schedule.

A sample answer and your facility’s answer

An example shows how information can be organized. It does not establish your ownership, staffing, insurance coverage or eligibility for an exemption. Use your facility’s evidence and check uncertain requirements.

Official sources to keep handy
